Your medical records are confidential and protected by law. If you would like your records transferred to a new provider or sent directly to you, please follow these steps:
Complete the Authorization Form
- Visit the and login with your Kent State email address and password.
- Once signed into the portal, select Forms.
- Once in the forms, select the Release of Protected Health Information form.
- Fill out all information on the Release of Protected Health Information form and submit the form.
- To transfer records to a new provider, include the provider's name, address and phone number.
